A 2021 lifespan review by Smith-Ryan and colleagues consolidated the evidence specifically on creatine for women's health, spanning adolescence, the reproductive years, pregnancy, and menopause. Their conclusion: creatine is a safe, effective strategy for supporting muscle, bone, and brain health across the female lifespan.1
Why Women May Start at a Deficit
Total-body creatine stores are partly determined by dietary intake and partly by endogenous synthesis — and both are influenced by sex hormones. Evidence suggests women tend to have lower intramuscular creatine stores than men, a difference that may relate to estrogen and progesterone.1 Lower baseline stores mean more room to benefit from supplementation, and several studies report that women respond to creatine even without the loading phases historically marketed to men.
This reframing matters. Creatine is not a "male" supplement that women can also use; it is a fundamental energy substrate, and women may simply have more to gain from restoring it.
Muscle and Bone: The Functional Payoff
The most consistent finding in women and older adults is on muscle and strength. Meta-analyses of resistance-training studies show that creatine, combined with training, produces greater gains in lean mass and strength than training alone — and this effect holds in older women.2,3 Muscle is not cosmetic; it is the tissue of independence. It determines how easily you climb stairs, carry groceries, and catch yourself from a fall.
Bone is the quieter story. Creatine's role in bone is less direct than its role in muscle, but it supports the energetic demands of bone-forming cells, and some evidence suggests creatine can enhance the bone benefits of resistance training in older adults.4 For women — who face a disproportionate burden of osteoporosis — this is a meaningful, if still developing, area of evidence.
